Scar revision is a surgical or non-surgical procedure performed to improve the appearance, texture, or function of a scar. Although scars cannot be completely removed, scar revision aims to make them less noticeable, flatter, softer, or better aligned with natural skin lines. It is commonly done for hypertrophic scars, keloids, widened scars, depressed scars, contractures, or cosmetically unacceptable scars. 

Surgical scar revision involves excising the old scar and carefully re-closing the wound with precise technique to achieve a finer, less visible scar. The incision is often placed along natural skin tension lines to improve cosmetic outcome. Techniques such as Z-plasty or W-plasty may be used to reorient scars, lengthen contracted scars, or break up straight scar lines. In cases of scar contracture, especially near joints, surgical release may improve movement and function.
